Senior Support Services
Community Living Parry Sound supports seniors who have a developmental disability in the following ways:
1. A day program for seniors offering in-house activities such as art and drama. This program also provides respite for families.
2. A community participation program which accesses activities within the community such as bowling and music.
3. Provides Selkirk contract positions allowing seniors to work in-house at the CLPS workshop or, in some cases, people may be able to work from home.
4. Senior Outreach Services – This service provides support for adults with a developmental disability who have recently moved into, or currently live in a Long Term Care Facility. CLPS provides support to help the person adjust to their new living environment and connect with services being offered within the facility.
The above services are intended to mirror the values of the larger CLPS organization and conform to its current strategic directions. This means that the services will be guided by an approach that is steeped in the ideal of community first as a cornerstone for people being able to participate more fully in all aspects of their community and the use of a person-centred service delivery model.
For planning purposes, CLPS will ensure a team approach to case management. CLPS will assume the lead agency role when multiple agencies are involved. The Community Inclusion and Support Facilitator will provide Service Coordination. Specifically the role of Community Inclusion and Support Facilitator will be to:
a) Promote and facilitate an inclusion based approach that links the person to community based services and supports
b) Promote and facilitate opportunities that connect each person to the community effectively and empowers them to advocate on their own behalf and increase their capacity and potential to do so.
