Child and Family Support Services within CLPS are intended to provide one seamless service approach for children between the ages of birth to 18 and their families in the following specific areas:
1. Family Support Services is for children (3-11) and their families where a developmental disability is evident.
2. Transitional Youth Services for adolescent aged children (12-17) and their families where a developmental disability is evident.
3. Respite Services for children and youth (birth to 18) where a child or youth with a developmental disability is residing.
Child and Family Support Services are intended to mirror the values of the larger CLPS organization and conform to its current strategic directions. This means that the services will be guided by an approach that is steeped in the ideal of ‘community first’ as a cornerstone for children and youth being able to participate more fully in all aspects of their community and the use of a person-centre service delivery model.
Case management will ensure a team approach for planning purposes, and CLPS will be the lead agency when multiple agencies are involved. The Community Inclusion and Support Facilitator will provide service coordination and may refer children or youth that have complex needs to Intensive Service Coordination through Hands the Family Help Network.
The role of the Community Inclusion and Support Facilitator:
a) to promote and facilitate an inclusion based approach that links the child/youth and/or family to community based services and supports
b) to promote and facilitate opportunities that connect each person to the community effectively and empower them to advocate on behalf of their family member and increase their capacity and potential to do so.
